Coosa Valley MRI participates in most major insurance companies’ plans and will submit claims to your insurance carrier on your behalf. You will receive two bills; one from Coosa Valley MRI for the technical services and one from Horizon Imaging for the professional reading services.
On the day of your exam please bring your insurance card(s), your photo ID and a list of your current medications. These will be scanned into your medical record. If you are covered by more than one insurance company please make us aware of your primary and secondary insurance provider. If there is a change in your insurance coverage at any time, please contact an Administrative Scheduling Representative to update your records. If your insurance is in your spouse’s name we will need their date of birth. If you are not the guarantor we will need the specified guarantor’s name, date of birth, and social security number. If the patient is a minor we will need the accompanying parent or guardians’ photo ID.
Our Administrative Team will work with your physician’s office in the event your insurance requires preauthorization.
Co-Payments and Deductibles
Co-payments and other balances are due on the day of your appointment. If you have questions, or need to confirm your co-pay/deductible requirements, please contact your insurance provider.
Filing Insurance Claims
As a courtesy we will verify your eligibility to make sure your provider will cover the MRI exam. In the event your insurance carrier does not pay the entire bill we will send a statement notifying you of any remaining unpaid balances.
